Registration Forms and Checklists

Below you will find a checklist of everything that needs to be done to complete program enrollment for returning and new students . You may view and print any form by clicking on the form name in the checklist.

Parents or guardians should complete the online registration (ideally with their student). Required forms must be printed, completed and signed, and then sent by U.S. Postal Service to the address below.

Summer Recording Workshop

  • Complete Online Registration form (new and returning students).

  • Pay tuition. A non-refundable deposit payment of $150 is required to hold a place in the workshop or parents may pay tuition of $485 when registering their student. Deposit and tuition fees may be paid by check, payable to Community Chorus Project, and mailed to the above address (please include student’s name in the memo line) OR paid online. (Please note, a 3% transaction fee has been added for online payments made by credit card.) Parents of new singers should not pay tuition until their student has been offered a place after their audition.

  • Print, complete and sign Program Agreement and Policies form.

  • Print, complete and sign Medical and Emergency Notification form.

  • Print, complete and sign Participant Agreement form.

  • Print, complete and sign (optional) Lunch Consent form that allows students to leave campus for lunch unsupervised..

Please note, we require your original signature. Program forms must be sent via U.S. Postal Servicel to the address above. Emailed or scanned forms will not be accepted. Deposit/tuition payment should accompany the mailed forms (if not previously been paid online).
